What does a medical scribe VA do?
Cloverpo Guides · October 2026
A medical scribe VA documents patient visits in your EHR while your provider focuses on the patient. The goal is simple: notes that are complete by the end of the day, not finished at the kitchen table.
How virtual scribing works
The scribe joins each visit remotely, usually through a secure video or audio connection set up by your practice. As the provider and patient talk, the scribe records the encounter in your EHR using your templates. After the visit, the provider reviews the note, makes any changes and signs it.
Some practices prefer that the scribe works from recorded dictation instead of joining live. Both approaches work. The right one depends on your workflow, your patients and your policies.
What a scribe typically documents
- The history of present illness and review of systems, as the provider describes them
- Exam findings the provider states during the visit
- The assessment and plan, in the provider’s words
- Orders, referrals and follow-ups, queued for the provider to approve
- Updates to problem lists, medications and allergies
What stays with your provider
A scribe documents. They don’t make clinical decisions, give medical advice, sign notes or finalize orders. Your provider reviews every note and remains responsible for its content. Clear rules on this protect your patients, your provider and the scribe.
Signs a scribe could help your practice
- Providers regularly finish charts after hours
- Notes are completed days after the visit
- Providers spend more time looking at the screen than at patients
- You’d like to see more patients without adding hours to the day
Privacy basics
Because a scribe hears patient conversations and works in your EHR, privacy has to be built in from day one:
- Give the scribe their own login with only the access they need. Never share provider logins.
- Many practices let patients know a scribe is present and offer to pause if a patient prefers. Follow your own policies and state rules.
- Make sure the scribe works in a private space where no one else can see or hear patient information.
- Put your data-handling expectations in writing in your agreement with your VA provider.
